Does a specimen have to be submitted for all classes?

Yes. In the US, you have to submit a specimen of use for each class of goods and services in your trademark application. Each specimen must show the mark in active use in connection with the specific goods or services in that class. A specimen that only shows use in one class does not satisfy the requirement for another.
