The USPTO fee for filing a Statement of Use electronically is $150 per class of goods and services. Paper submissions cost $200 per class.

If you need to extend the deadline before filing the Statement of Use, each six-month extension request costs $150 per class for electronic filings and $250 for paper filings. Up to five extensions are available, giving a maximum of 36 months from the Notice of Allowance date to file the Statement of Use.

Please note these are government fees paid directly to the USPTO. If you file through a representative, additional fees for their services will apply.
